Our office is located on the corner of 4th street and Hwy. 80 across from Good Shepherd Medical Center in Longview. 

Our office is in Medical Plaza II on the 4th floor, Suite 4009.  From our free patient parking garage take the east entrance into the building, turn left down the first hallway and take the elevators at the end of the hallway to the 4th floor.
